CBT Contract

Following assessment we have agreed on CBT as your therapy option . Some general information about CBT can be found here

In addition to the information shared with you when you referred to the service (our terms and condition can be found here, I would like to share some further information about what you can expect from CBT, your therapist and what is expected from you.

What I expect from you

CBT is a structured ‘doing’ therapy and I expect you to get involved with understanding your difficulties, to engage in homework and to make changes. We will only see each other for a small amount of time each week and it is the changes made outside of the session that will have the biggest impact on your progress in therapy.

For therapy to be effective sessions need to be regular and therapy needs to be prioritised; therefore, if there are 2 occasions where you do not attend or you cancel appointments with less than 24 hours’ notice, you will be discharged. If you are regularly cancelling appointments or turning up late this can also impact on the effectiveness of therapy and will result in being discharged from the service.

Progress will also be reviewed every 3/4 sessions. If it is felt that CBT is not the best approach for you, that now is not the best time to make changes, or you are struggling to complete homework tasks then we would discuss other options and bring CBT to an end.

CBT is time-limited and we only offer short term therapy within Impact, therefore we need a clear goal to work on to help you get the most from your sessions. We are unlikely to have time to work on multiple problems, which is why we ask you to think about these goals in advance.

Before attending your first session, please consider and fill in the following information which will help us to decide what area you want to work on:

List of problems you are experiencing rating each one out of 10 (with 0 being ‘this problem affects me the least’ and 10 being ‘this problem affects me the most’). This will help us to decide what to prioritise in therapy

Problem list (please list as few or as many as you feel relevant) – please make some notes and bring to your first session

We will discuss this further in your first sessions and make a plan for how to best to utilise our time together.

What you can expect from me

I will offer therapy appointments lasting between 50-60 minutes which will be offered at the same time and day each week. If I can’t make a session, I will let you know as soon as possible.

I will use CBT formulation to help make sense of your difficulties and share evidence-based techniques to help make changes to the way you think, feel and act, with the aim of improving the way you feel and to reduce unwanted symptoms.

I have regular supervision and keep up to date with training and research and will do my best to help you and adapt to your needs. We are not a specialist service but can make some adaptations to treatment when needed.
